We understand the contribution each and, every Employee makes to our accomplishments and so our goal is to provide a comprehensive program of competitive benefits to attract and retain the best Employees available. Through our benefits programs, we strive to support the needs of our Employees and their dependents by providing a benefit package that is easy to understand, easy to access and, affordable for all of our Employees. This brochure will help you choose the type of plan and level of coverage that is right for you.

MEDICAL COVERAGE

Two (2) medical plans through Aetna: AVN HMO and OAMC POS. You pay your share of the premium through pre-tax payroll deductions.

MEMBER CARE OPTIONS

Access affordable care options, including Teladoc, MinuteClinic®, and urgent care, using the Aetna HealthSM app.

DENTAL COVERAGE

Dual PPO plan offered through Delta Dental. You pay your share of the premium through pre-tax payroll deductions.

VISION COVERAGE

Aetna Vision plan covers exams, eyewear, and offers discounts. Premiums are paid pre-tax through payroll deductions.

HEALTH CARE PREMIUMS

Select from flexible medical, dental, and vision coverage options through Aetna and Delta Dental. Semi-monthly pre-tax premiums ensure affordable coverage for you, your spouse/domestic partner, children, or family.

COMPANY PAID BASIC LIFE AND AD&D

Company offers benefit-eligible employees Life and Accidental Death and Dismemberment (AD&D) insurance. Employees are encouraged to designate beneficiaries during online enrollment.

VOLUNTARY WORKSITE BENEFITS

The company offers voluntary Accident Insurance through Reliance Standard, covering unexpected injuries with options for family coverage. The plan includes a $50 annual wellness benefit per insured, with premiums paid via payroll deductions.

VOLUNTARY LIFE

Select from voluntary Life and AD&D insurance options with coverage for yourself, your spouse/domestic partner, and children, featuring guarantee issue limits and affordable semi-monthly premiums.

EMPLOYEE ASSISTANCE PROGRAM

Company provides benefit-eligible employees access to Mutual of Omaha’s Employee Assistance Program, offering support for personal, legal, financial, and emotional challenges, plus additional services like travel and identity theft assistance.
